The Care Team Lead will supervise and lead a team of Community Health Partners (CHPs) to impact birth outcomes, racial equity, and member experience. This role involves coordinating interdisciplinary care team operations, providing guidance on engagement, care planning, and longitudinal care. The position requires 3-4 years of leadership experience in supervising social workers, community health workers, or care coordinators, along with experience in leading interdisciplinary teams and working with pregnant/postpartum individuals with behavioral health diagnoses.
Must have:
  • Provide supervision and leadership for a team of Community Health Partners (CHPs).
  • Lead the operations of an interdisciplinary care team, coordinating team time.
  • Provide guidance, coaching, and training on engagement, care planning, and longitudinal care.
  • 3-4 years of leadership experience providing supervision to social workers, community health workers, or care coordinators.
  • Experience leading day-to-day operations of interdisciplinary teams.
  • Experience working with pregnant and/or postpartum individuals with behavioral health diagnoses.
  • Proof of COVID-19 vaccine or approved exemption.
  • Complete medical clearance requirements (MMR, Hepatitis B, Varicella, TB screen) or approved accommodation.

Job Description:

The Care Team Lead works in a radically different model of healthcare designed to meaningfully impact birth outcomes, racial equity and experience in our member population.

Counties: Worcester, Middlesex, Suffolk, Essex, Barnstable, Norfolk, Plymouth, Bristol, Barnstable.

Language (Nice to have) Spanish, Vietnamese, Cantonese, Cape Verdean Creole, Vietnamese

Responsibilities:

  • Provides supervision and leadership for a team of Community Health Partners (CHPs), supporting them in meeting population level expectations.
  • Leads the operations of an interdisciplinary care team, directly coordinating team time for each of the care team members.
  • Provides guidance, coaching, and training around engagement, care planning, and longitudinal care.

Work Experience:

  • 3-4 years of leadership experience providing supervision to social workers, community health workers, or care coordinators
  • Experience leading the day-to-day operations of interdisciplinary teams
  • Experience working with pregnant and/or postpartum individuals with behavioral health diagnoses

The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location. The expected salary range for this position is:

88,358.00 - 116,127.00 Salary
