The Director will be responsible for developing and maintaining data systems to track contract performance for our CHC partners to support high quality and equitable care for the patient populations seen at CHCs. Using this data system, the Director will support performance management of CHC partners in Aledade ACO networks by surfacing opportunities for improvement based on data.
This role requires experience working with Federally Qualified Health Centers or prior experience holding a leadership role at an FQHC, as well as understanding population health analytics and value-based care principles. The Director of CHC Performance will understand the environment and complexities under which CHCs provide value-based care (stakeholders, policies), and will be able to lead the development of new programs and resources for CHC performance improvement based on data analytics and his/her comprehensive understanding of CHCs.
The Director of CHC Performance will be a key leader for the Aledade CHC Center of Excellence, designed to enhance Aleadade’s value-based care support to CHCs by developing and managing programs that training, educate and facilitate VBC success for CHCs (affordability, risk, quality metrics), as well as adapting Aledade’s core service offerings for CHCs, all through a community-centric approach.
The Director is expected to work closely with leaders from other teams at Aledade to develop strategies and solutions that support organizational key result (KR) performance outcomes, and s/he must also be comfortable speaking about CHC performance in settings with CHC leaders.
This role works with both internal and external partners, and requires strong organizational, communication, and analytical skills. The Director will need to synthesize feedback and comments and translate the information into actionable next steps to ensure the successful execution and achievement of program goals.
We are flexible with respect to geographic location, and the ideal candidate will be comfortable working remotely within the US, with periodic travel required for internal planning meetings and in-person CHC events.
