Pomelo Care is seeking a full-time Pediatric Triage RN to provide patient-centered clinical care to pediatric patients. This role involves assessing urgent concerns, triaging patients, conducting clinical and psychosocial assessments, addressing care barriers, and educating patients on remote monitoring. The ideal candidate is a BSN-prepared Registered Nurse with 3+ years of pediatric outpatient or hospital experience, capable of working remotely, weekends, and holidays, and committed to health equity.
Good To Have:

Must Have:
  • Registered Nurse, BSN prepared, current unrestricted multi-state (compact) license
  • 3+ years experience in an outpatient practice or hospital, serving pediatric populations
  • Ability to assess urgent pediatric concerns and proactively triage patients
  • Proficiency in following evidence-based clinical protocols
  • Skill in ongoing clinical and psychosocial assessment of new pediatric patients
  • Ability to identify and address barriers to care
  • Experience in reducing care gaps through frequent patient engagement
  • Capability to assess and educate patients on remote health monitoring equipment
  • Competence in ongoing surveillance and timely response to abnormal diagnostic results
  • Adept at timely documentation and escalation to multidisciplinary teams
  • Willingness to work Monday through Friday, 11am-8pm EST, with rotating weekends and holidays

Key responsibilities of the role

Your North Star: Provide and facilitate amazing patient-centered clinical care to our pediatric patients.

\*\*We are currently hiring for the following full-time shift: Monday through Friday, 11am-8pm EST, with a rotating weekend requirement. Please note that holiday coverage is also expected on a rotating basis.\*\*

Reporting to the Director of Nursing, your key responsibilities will include:

  • Assessment of urgent pediatric concerns and proactively triaging patients to support appropriate utilization of Pomelo Care and/or emergency services
  • Following evidence-based clinical protocols in the assessment and triage of pediatric patients
  • Ongoing clinical and psychosocial assessment of new pediatric patients, providing reassurance and building rapport
  • Identifying and addressing barriers to care that have been identified by patients and/or the care team.
  • Reducing care gaps (missed appointments, medication management, etc.) by frequent engagements with patient
  • Assessing the need for and educating patients on the equipment they will use to monitor their health remotely
  • Ongoing surveillance and timely response to abnormal diagnostic results (labs, radiology, etc.)
  • Timely documentation of all care/interactions and escalating to appropriate multidisciplinary teams, as needed
  • Supporting the development of programs and product by providing user feedback

Who you are

  • Registered Nurse, BSN prepared, current unrestricted multi-state (compact) license
  • Have 3+ years experience in an outpatient practice or hospital, serving pediatric populations
  • Flexible and agile thinker who embraces change
  • Are internet-connected, able to work remotely via video, phone and text
  • Willing to travel occasionally (infrequent)
  • Willing to work weekends and holidays

Individual pay decisions are ultimately based on a number of factors, including qualifications for the role, experience level, skillset, geography, and balancing internal equity. A reasonable estimate of the current salary range is $78,000-$83,000.
