Job Description

Ability Action Australia is seeking Behaviour Support Practitioners of all experience levels for their South Perth team. The role involves supporting NDIS participants in the South Perth Region by identifying needs, developing evidence-based support plans, setting meaningful goals, and building capacity for participants and their support networks. Candidates will work collaboratively within a multidisciplinary team, with opportunities for mentoring.
Good To Have:
  • Completed a degree level qualification as an allied health professional.
Must Have:
  • Support NDIS participants to identify needs through assessment and person-centred planning.
  • Evaluate, develop and deliver evidence-based support plans within a complex support space.
  • Develop meaningful goals and practical strategies as per the positive behaviour support framework.
  • Ensure individual plans reflect participants' goals, passions and strengths.
  • Build the knowledge, capacity and strengths of those who support participants in their lives.
  • Work collaboratively within a multidisciplinary team, including mentoring less experienced Behaviour Support Practitioners.
  • Report writing experience.
  • Excellent time management and organisational skills to manage a varied caseload.

Ability Action Australia's South Perth team are looking for Behaviour Support Practitioners of all experience and suitability levels. Whether you are just starting your career or have suitability levels at Core, Proficient or Advanced, we want to hear from you!

Successful candidates will be providing supports to NDIS participants living in the South Perth Region.

Responsibilities will include:

  • Supporting NDIS participants, along with their families and/or carers, to identify their needs through assessment and person-centred planning.
  • Evaluating, developing and delivering evidence-based support plans within a complex support space.
  • Developing meaningful goals and practical strategies as per the positive behaviour support framework, with a commitment to eliminate restrictive practices, in the home and community
  • Ensuring individual plans reflect participants' goals, passions and strengths.
  • Building the knowledge, capacity and strengths of those who support participants in their lives - family, carers, friends, etc.
  • Working collaboratively within a multidisciplinary team, including mentoring less experienced Behaviour Support Practitioners.

Qualifications

  • Completed a degree level qualification as an allied health professional is desired
  • Report writing experience
  • Excellent time management and organisational skills to manage a varied caseload
  • Proactive attitude, motivated by improvement and the opportunity to test and learn from new ideas
  • Genuine passion for working in the disability sector
