The Virtual Urgent Care (VUC) Physician provides care to Cityblock members as part of an innovative care model, working on the Mobile Integrated Care team. This role involves 24/7 virtual urgent care, deploying Community Care Clinicians to members’ homes, developing treatment plans via phone/video, and overseeing advanced practice providers. Responsibilities include delivering acute care for complex populations, supervising clinical care, preventing unnecessary ER visits, and collaborating with longitudinal care teams. The role operates in rotating shifts, including days, evenings, weekends, and holidays, for 20 hours per week.

  • Working closely with the Community Care Clinicians to plan next steps
  • Closing the loop with the member’s primary care team
  • Operates in rotating shifts including days, evenings, weekends, and holidays
  • 20 Hours per week
  • Delivers acute care for a complex population of members in a collaborative, interdisciplinary team model
  • Effectively supervises and provides leadership of clinical care within interdisciplinary team, ensuring highest quality of care
  • Ensures members receive appropriate care for acute and chronic diseases
  • Collaborates effectively with other Cityblock providers and longitudinal care teams
  • Prevents unnecessary emergency room visits
  • Timely documentation and loop closure
  • Active participation in Virtual Urgent Care and ED at Home model development, redesign, and innovation
  • Works with flexibility for 8-12 hour shifts, including some weekend day and holiday day call coverage

The Virtual Urgent Care (VUC) Physician provides care to Cityblock members as part of our innovative care model. They work on the Mobile Integrated Care team which operates 24/7 virtual urgent care and can deploy Community Care Clinicians (typically paramedic, EMT, RN, or MA trained) to members’ homes in response to urgent member clinical and behavioral needs. The VUC Physician, together with our team of virtual emergency medicine PAs/NPs, develops treatment plans and addresses acute needs via phone or video, and must be able to deploy our Community Care Clinician ED at Home field team to implement assessment and treatment at the member’s home. They are responsible for collaborating with and overseeing advanced practice providers (NPs/PAs) during virtual visits, working closely with the Community Care Clinicians to plan next steps, as well as closing the loop with the member’s primary care team.

This role operates in rotating shifts including days, evenings, weekends, and holidays.

20 Hours per week

Preferably 3 or more of the following state licenses:

Il, IN, MI, OH, NY, MA NC, FL

Responsibilities:

Work Experience:

  • 3+ years in clinical practice post-residency training.
  • Experience in acute clinical care, managed care or value-based care operations
  • Experience providing comprehensive care management, chronic disease management, urgent home-based and community-based urgent care visits
  • Experience liaising with other providers around medical care, behavioral health, long term service and support needs, and the provision of palliative care
  • Has interest in working in a highly collaborative, integrated and multidisciplinary setting
  • Quiet private environment for virtual visits

Education:

  • Graduate from an accredited medical college (within the United States/Canada) or from an unaccredited medical college (outside of the United State/Canada).
  • Board Certified in Emergency Medicine
