Job Description:

The Referral Coordinator manages the referral process from end-to-end including liaising with and completing the necessary paperwork for external care providers/facilities (e.g., skilled nursing facilities, rehab facilities, home PT/OT, complex DME), health insurance benefits programs (e.g. gym membership, nutrition/cooking classes, prescription medication delivery), and government/community support options (e.g. subsidized housing, legal support, food delivery programs).

Responsibilities:

  • Communicates with members and external agencies regarding referrals made on their behalf.
  • Initiates and manages referrals based on member needs, ensuring effective communication throughout the process.
  • Coordinates referrals with both internal and external stakeholders to streamline member care.
  • Collaborates/coordinates with external PCPs to facilitate referrals to specialists and home health agencies.
  • Leverages care facilitation tools, electronic health records, and scheduling platforms to document member interactions, gather data, organize information, manage tasks, and maintain effective communication with team members, members, and community resources.

Work Experience:

  • 1+ years of experience in a healthcare, clinical, or community services setting with a focus on care coordination and resource navigation.
  • 1+ years demonstrated experience in managing complex referral processes.
  • Experience with Athena preferred.

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, retirement benefits, participation in the company’s equity program, paid time off, including vacation and sick leave. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location. The expected salary range for this position is:

$21.00 - $24.00 Hourly
