The Advanced Community Care Clinician (ACCC/AC3) delivers medical care to Cityblock members in their communities, including homes, shelters, and public places. This role facilitates virtual provider visits, performs clinical tasks like IV insertion, medication administration, EKG, and phlebotomy within scope. The ACCC/AC3 connects members to Cityblock services, managing various visit types such as emergency at home, facilitated visits, outreach, specimen collection, and MIC-only follow-ups. Each shift begins with equipment review and visit scheduling.
Must Have:
  • Delivering medical care in the communities where Cityblock members live
  • Facilitating virtual provider visits with Cityblock members
  • Following medical orders, performing clinical skills and tasks within their scope, training, and policy (e.g., IV insertion, medication administration, EKG, Phlebotomy)
  • Connecting members to Cityblock services including medical providers
  • Managing primary visit types: Emergency at home, Facilitated Visit, Outreach, Specimen Collection, MIC Only Followup
  • Reviewing equipment, communications, and scheduled visits at the start of each shift
  • Current EMT Paramedic certification
  • 4+ Years Work Experience
  • Completing Cityblock’s medical clearance requirements (immunity to MMR, Hepatitis B, Varicella, TB screen)

Job Description:

The Advanced Community Care Clinician is a clinical role responsible for delivering medical care in the communities where Cityblock members live in homes, shelters, offices, public places or in the street. The ACCC/AC3 is primarily responsible for facilitating virtual provider visits with Cityblock members and following medical orders, performing clinical skills and tasks within their scope, training, and policy, which may include IV insertion, IV/IM/SQ medication administration, 12 EKG, Phlebotomy, etc. (Dependent upon local legal regulations)

Requirements:

The Advanced Community Care Clinician is pivotal in connecting members to Cityblock services including medical providers and access to all Cityblock services. There are several primary visit types they will be responsible for:

  • Emergency at home: Acute illness facilitated provider visit
  • Facilitated Visit: Longitudinal care team facilitated provider visit
  • Outreach: Independent visit to find hard to reach members
  • Specimen Collection: Independent visit to obtain tissue samples for the lab.
  • MIC Only Followup: Independent visit with a specific set of visit goals
  • The Advanced Community Care Clinician begins each shift by reviewing their equipment, communications, and scheduled visits.

Certification Required: Current EMT Paramedic certification required.

Work Experience: 4+ Years

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, retirement benefits, participation in the company’s equity program, paid time off, including vacation and sick leave. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location. The expected salary range for this position is:

30.721 - 32.00 hourly
