Job Summary
We are seeking a full-time Care Manager/CNA for a temporary position, lasting 8 to 12 weeks, with the potential for permanent employment. This role is crucial for providing coverage during a medical leave through February 2026. The Care Manager will assist residents with all aspects of daily life, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities, as outlined in their care plan.
Must Have
- Provide resident care including bathing, dressing, toileting, and feeding as outlined in care plan
- Maintain the safety, comfort, dignity, and confidentiality of all residents
- Escort residents to meals and activities
- Ensure all assigned residents are clean, neatly dressed and groomed, safe, and fed during shifts
- Document all services provided to assigned residents during your shift
- Report any change of condition in a resident immediately to your supervisor
- Certified Nursing Assistant and/or CHHA certification
- High School Diploma
- Knowledge and experience in working with the elderly population

Care Manager (Temporary – 8 to 12 weeks, Possible Permanent)
Salary Range: $21.00 - $21.20 Hourly
We’re looking for a full-time Care Manager/CNA to provide coverage during a medical leave through February 2026. This role is temporary to start, with the possibility of becoming permanent.
Note: This position is intended to backfill an employee on medical leave. The temporary hire will remain in place until the employee returns or the leave/retirement is finalized.

Job Description:

Qualifications
While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
Certified Nursing Assistant and/or CHHA certification plus High School Diploma
Knowledge and experience in working with the elderly population, preferred dementia experience
Reports to the Health & Wellness Care Director.
