Community Health Partners support integrated social care for members with complex health needs, addressing social, behavioral, and physical factors. This role involves meeting members in various community settings, identifying and overcoming barriers to optimal health, and coordinating care for a caseload. Candidates need 2+ years of experience in care coordination, community health, or health promotion to effectively manage member needs and contribute to improving health outcomes in underserved communities.
Must Have:
  • Meet with members in the community (home, short-term nursing facility shelter, hospital) as needed.
  • Care for members, identifying and addressing their barriers to and social influences on optimal health.
  • Coordinate care for a case load of members.
  • 2+ years of progressive experience in care coordination, community health, health promotion, or a related field.
  • Provide proof of COVID-19 vaccine or approved exemption.
  • Complete medical clearance requirements (immunity to MMR, Hepatitis B, Varicella, and a TB screen) or approved accommodation.

Job Description:

Community Health Partners support integrated social care for members with complex health needs (social, behavioral and physical).

Responsibilities:

  • Meets with members in the community (home, short-term nursing facility shelter, hospital) as needed, including as an extension of the care team
  • Cares for members, identifying and addressing their barriers to and social influences on optimal health.
  • Coordinates care for a case load of members, identifying and addressing their barriers to and social influences on optimal health.

Work Experience:

  • 2+ years of progressive experience in care coordination, community health, health promotion, or a related field.

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, retirement benefits, participation in the company’s equity program, paid time off, including vacation and sick leave. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location. The expected salary range for this position is:

$26.22 - $29.13 Hourly
