Carda Health is seeking a meticulous Chronic Care Management Assistant to manage and organize medical data for Cardiopulmonary Rehab patients. The role involves ensuring patient data privacy and accuracy in compliance with HIPAA, maintaining medical records, and responding to inquiries from healthcare providers. Responsibilities include converting patient data into EMR systems, collecting and reviewing Past Medical History, performing medication reconciliation, generating reports, indexing files, inputting patient data, assisting with coding and billing, and providing general administrative support. The role also involves working with founders to innovate the digital care delivery model. A passion for telehealth and improving patient access to life-saving care is essential.
Must have:
  • Bachelor's degree in Clinical Exercise Physiology/Exercise Science or Nursing
  • Minimum 2 years of experience in a medical office setting
  • Proficiency with medical records systems and software
  • Excellent attention to detail and accuracy in data entry
  • Knowledge of medical terminology
  • Strong organizational and communication skills
  • Familiarity with HIPAA regulations
Good to have:
  • 2+ years in a similar role

  • ACSM Clinical Exercise Physiologist Certification

In this role, you will

  • Ensure the privacy and accuracy of patient information in compliance with HIPAA regulations.
  • Maintain medical records accurately and timely, ensuring proper organization and confidentiality.
  • Respond to inquiries from healthcare providers regarding patient records and information.
  • Obtain patient information from healthcare professionals and convert data into EMR systems for Chronic Care Management programs
  • Help to collect, organize, and review Past Medical History and to ensure the best patient care options
  • Knowledge of cardiac and pulmonary medications and the ability to do a medication reconciliation
  • Generate reports and summaries from medical records for physicians and administrators as requested
  • Index and cross-reference files for easy retrieval and efficient record management.
  • Input patient data into the medical records system, including updates to demographics, medical history, and treatment plans.
  • Assist with coding and billing processes to support accurate reimbursement and compliance.
  • Provide general administrative support and assist with other tasks to maintain smooth operations in the medical office.
  • Work directly with founders to innovate and improve our cutting edge digital care delivery model.

Work Skills That We Look For:

  • A Bachelor's degree in Clinical Exercise Physiology / Exercise Science or Nursing
  • Minimum of two years of experience in a medical office setting, preferably in cardiac and pulmonary rehab.
  • Proficiency with medical records systems and software.
  • Excellent attention to detail and accuracy in data entry and record management.
  • Knowledge of medical terminology and understanding of medical record documentation.
  • Strong organizational, problem-solving, and communication skills.
  • Familiarity with HIPAA regulations and commitment to patient privacy and confidentiality
