The Health Navigator Specialist will support newly arrived refugees by providing culturally and linguistically appropriate healthcare navigation services. This includes increasing health literacy, assisting with healthcare access, and providing personalized support to mitigate barriers to medical and emotional wellness services. The role involves developing and implementing individualized Plans of Care (POC), conducting welfare checks, coordinating referrals, and facilitating health education and wellness groups.
Must have:
  • Bachelor’s degree in a related field or equivalent experience
  • Minimum 2 years healthcare navigation, case management experience
  • Knowledge of US healthcare system, Medi-Cal
  • Experience working with refugee and immigrant communities
  • Proficiency in Microsoft Office Suite and EHR systems
Good to have:
  • Proficiency in a second language (Dari, Pashto, Russian, Ukrainian)

ORGANIZATION SUMMARY

This position is reliant upon funding and may be subject to modification or termination based on resource availability.


POSITION SUMMARY:
The Health Navigator Specialist will play a vital role in supporting newly arrived refugees by providing culturally and linguistically appropriate healthcare navigation services. This position is responsible for increasing health literacy, assisting with healthcare access, and providing personalized support to mitigate barriers to medical and emotional wellness services. The Health Navigator will develop and implement individualized Plans of Care (POC), conduct welfare checks, coordinate referrals, and facilitate health education and wellness groups. 

ROLE & RESPONSIBILITIES:

    • Health Literacy & Education:   
    • Conduct outreach, training, and educational sessions to improve refugee understanding of the healthcare system, preventive care, and emotional wellness.   
    • Reduce stigma surrounding healthcare and mental health services through culturally sensitive engagement.   
    • Deliver quarterly community-based health education and wellness groups tailored to the refugee population.   

    • Healthcare Navigation & Coordination:   
    • Assist participants in enrolling in Medi-Cal or Refugee Medical Assistance (RMA) and selecting appropriate managed care plans.   
    • Schedule, track, and facilitate attendance for Refugee Health Assessment Program (RHAP) appointments, including arranging transportation.   
    • Provide patient records to healthcare providers and offer telehealth education when necessary.   
    • Serve as a liaison between clients and healthcare providers to ensure timely and appropriate care.   
    • Provide transportation to health appointments as needed and coordinate rides through Medi-Cal once clients are approved for health coverage.

    • Plan of Care (POC) Development & Case Management:   
    • Develop individualized POCs for RHPP participants within 10 business days of program enrollment.   
    • Identify medical and mental health needs, provide referrals, and ensure access to healthcare services.   
    • Review and update POCs regularly, ensuring alignment with participant goals and evolving health conditions.   
    • Update and maintain client information in protected, online databases (Dynamics, Teams, etc.)   

    • Triage, Assessment, & Emergency Coordination:   
    • Utilize the RHS-15 tool to assess mental health distress and prioritize care needs.   
    • Facilitate urgent and specialized medical care referrals as needed.   
    • Perform monthly welfare checks to monitor participant well-being and address emerging healthcare challenges.   

    • Health Education & Wellness Group Facilitation:   
    • Organize and lead health education and wellness group sessions to promote physical and emotional well-being.   
    • Develop culturally tailored materials and resources to support refugee health education.   
    • Utilize various formats, including in-home classes, virtual sessions, and health videos in native languages.   

    • Advocacy & Support Services:   
    • Assist clients in navigating healthcare-related paperwork, scheduling appointments, and understanding treatment plans.   
    • Provide interpretation support for provider interactions when necessary.   Empower refugees to actively participate in healthcare decisions and advocate for their own well-being.   

    • Record Management & Reporting   
    • Maintain accurate case files and report client progress through case notes 
    • Ensure compliance with grant documentation and funding requirements   
    • Conduct case file reviews for program improvement   


    • Collaborate successfully across World Relief Sacramento staff and departments, as well as with external service providers to ensure holistic case management   
    • Attend meetings as instructed by supervisor   
    • Participate in on-going training to improve service delivery and ensure compliance   
    • Other duties as assigned 

JOB REQUIREMENTS:

    • Mature and personal Christian faith
    • Committed to the mission, vision, and values of World Relief
    • Desire to serve and empower the Church to impact vulnerable communities
    • Able to affirm and/or acknowledge World Reliefs Core Beliefs, Statement of Faith, Christian Identity and National Evangelicals For the Health of The Nation document
    • Bachelor’s degree in public health, social work, healthcare administration, or a related field (or equivalent experience).   
    • Minimum of 2 years of experience in healthcare navigation, case management, refugee resettlement, or community health services. 
    • Knowledge of the U.S. healthcare system, Medi-Cal, managed care plans, and refugee health policies.   
    • Experience working with refugee and immigrant communities, with an understanding of trauma-informed care. 
    • Proficiency in Microsoft Office Suite and electronic health record (EHR) systems.   
    • Local travel and transportation of program participants required, and use of personal car 

PREFERRED QUALIFICATIONS:

    • Proficiency in a second language: Dari, Pashto, Russian, Ukrainian (preferred).   
    • Strong interpersonal and communication skills, with the ability to build trust with diverse populations. 
    • Ability to work collaboratively in a multidisciplinary team and with external healthcare providers.
